A complimentary File of Life with a built-in magnet is available upon request at the Fearrington Cares Center. The File of Life provides first responders with quick and accurate information about your medical history in case you are unable to provide it yourself.
A File of Life form should be completed by each member of your household. This information should be updated each time your medical history changes. You can download new forms for your File of Life by selecting one of the following options:
- Online fillable pdf (fill out the form online and then print)
- Print and fill (print the form and then complete it by hand)
